D.O.T Exams
To prepare for your D.O.T. exam, please call the clinic to schedule an appointment and ensure you bring a valid photo identification, which is required for the visit. You will also need to provide your full medical records, contact information for your doctors, and a comprehensive list of all prescription and over-the-counter medications including their strengths and dosages. Depending on your specific health needs, please bring any eyeglasses, contacts, or hearing aids required for driving. If you have specific medical conditions, you must provide:

A copy of your most recent stress test (for heart conditions).

A copy of your CPAP sleep history (for diagnosed sleep apnea).

A doctor’s letter if you use Adderall or Vyvanse confirming the medical necessity, the stability of your condition and dosage, and that the medication does not impair your driving.
In the three days leading up to your exam, it is recommended to maintain a low-salt, high-potassium diet. Finally, immediately before your appointment, please drink a glass of water, as you will be required to provide a small urine sample during the evaluation.
